Welcome to the family! We are always happy to have new patients join our practice and look forward to seeing you soon. Please plan to arrive at least 15 minutes before your appointment time to complete your new patient forms. For your upcoming appointment, please be sure to bring your current insurance card, photo identification, medications and any existing glasses you currently wear (regular, computer, sunglasses, reading glasses, older spare glasses, etc.). To simplify the administrative part of your appointment, we are happy to offer new patient paperwork online for those patients interested in completing the form in advance.
